Please complete one form per formula, even if your feedback is the same across multiple products. For example, do not group items like “Shampoo and Conditioner” into a single form. Each product and formula number must have its own individual submission.

Sample Review Process

  • If your sample is approved: Simply fill out the required fields and submit the form. Our team will follow up with the next steps.
  • If your sample is not approved: Please provide detailed feedback explaining why. The more specific your comments, the better our Research & Development team can refine the formula to meet your expectations.
